The heartland of the title is rural Wyoming in the early 20th century, where dwells taciturn, reclusive Scottish farmer Rip Torn. Conchata Ferrell arrives at the Torn spread to work as his housekeeper. Paid near-starvation wages, Ferrell continues working day and night, hoping to use her savings to ensure a bright future for her 10-year-old daughter Megan Folsom. Touched by her diligence, Torn slowly falls in love with Ferrell, and after seeing the woman and her child through the torturous Wyoming winter, he marries her - but their union gets off to a shaky start and threatens to buckle, thanks to several unforeseen casualties. In the series premiere, Nathaniel is on track to become the new chief surgeon, a position currently held by Bart. One of Nathaniel's current cases is a teenage girl in desperate need of a new heart. After a woman is fatally injured in a car accident, her heart becomes available and is transplanted into the girl. Afterwards, the donor's husband wants to see the girl who got his wife's heart, despite very strict protocols preventing the hospital from giving him any information about her. Meanwhile, a grieving grandmother doesn't want her deceased grandson's organs to be donated, despite pleas from Kate, whose job it is to obtain family consent. And Thea gets into trouble trying to steal condoms from a drug store.

A woman with pulmonary problems desperately needs a liver, or her heart will fail. When a liver finally becomes available in West Virginia, Kate and Jonas drive there to procure it--but racial tensions delay them. Meanwhile, after another patient who has long been on the waiting list goes to Mexico for an illegal transplant, the donor liver fails, and she ends up in worse condition than before. An ethical dilemma occurs when Nathaniel is told that the woman cannot get another transplant because of what she did. Also, Kate tries dating again, going to dinner with a technical-support guy.

Meanwhile at the hospital, one of the potential organ recipients chooses to share the liver he needs with another patient--but when the tables are turned, the other patient isn't quite as giving. Also, Kate must train a new coordinator, and Thea continues to push for independence.
